The HIV/TB Mobile Nurse (SECHN)

The HIV/TB Mobile Nurse (SECHN) will provide high-quality, patient-centred nursing services through the Mobile HIV/TB Van, expanding access to integrated HIV and TB screening, prevention, counselling, referral, linkage to care, and treatment support for underserved communities in Kono District. The role is responsible for delivering safe outreach nursing care, identifying presumptive TB and HIV cases, supporting specimen collection and diagnostic pathways, strengthening adherence and follow-up, and ensuring that clients are linked promptly to appropriate facility- and community-based services.

The post holder will work as part of a multidisciplinary mobile team and will collaborate closely with community health workers, facility teams, the District Health Management Team, and national program structures to deliver respectful, confidential, and stigma-free services aligned with national HIV, TB, infection prevention, and safeguarding standards.

Essential Duties and Responsibilities:

  • Conduct client registration, triage, nursing assessments, and prioritise clients requiring urgent clinical care.
  • Perform integrated TB and HIV screening using approved national guidelines and outreach protocols.
  • Support mobile diagnostic services, including HIV testing, chest X-ray coordination, sputum collection, and specimen management within the scope of practice.
  • Identify presumptive TB, HIV, and TB/HIV co-infection cases, ensuring timely referral, linkage to care, and follow-up.
  • Provide basic nursing care, emergency response, patient monitoring, and treatment adherence support during outreach services.
  • Deliver confidential counselling and health education on TB, HIV, infection prevention, early diagnosis, treatment adherence, and stigma reduction.
  • Collaborate with community health workers, local leaders, and health facilities to implement community mobilisation, active case finding, contact investigation, and health promotion activities.
  • Ensure compliance with infection prevention and control (IPC) standards, including PPE use, safe specimen handling, environmental controls, and infectious waste management.
  • Maintain accurate, timely, and confidential clinical documentation, including screening outcomes, referrals, counselling, specimen collection, and program reports.
  • Promote quality, patient safety, and professional nursing practice by participating in quality improvement initiatives, safeguarding, multidisciplinary teamwork, and continuous service improvement.
  • Participate in stock monitoring, inventory reconciliation, and timely communication of supply gaps that may affect service delivery.

Education and Work Experience Requirements:

  • State Enrolled Community Health Nurse (SECHN) certificate or equivalent qualification recognised in Sierra Leone.
  • Valid practising license from the Nursing and Midwifery Board of Sierra Leone.
  • Minimum of two years of clinical experience in primary health care, HIV, TB, community health, infectious disease, or mobile outreach services.
  • Demonstrated ability to provide respectful, confidential, patient-centred nursing care in community and facility settings.
  • Knowledge of national HIV, TB, TB/HIV, infection prevention, and referral guidelines, or willingness to rapidly build competency in these areas.
  • Strong counselling, communication, documentation, teamwork, and community engagement skills.
  • Commitment to social justice, human rights, safeguarding, and PIH’s mission and values.
  • Must NOT hold dual employment, neither a PIN code from private nor MOH
  • Previous experience in HIV testing services, TB screening programs, contact investigation, adherence support, or active case finding.
  • Experience working with the National Tuberculosis and Leprosy Programme, national HIV program structures, DHMTs, or community health systems.
  • Experience delivering services through mobile clinics, rural outreach, or hard-to-reach community programs.
  • Familiarity with electronic data tools, patient registers, commodity tracking, and routine program reporting.
